We investigated the characteristics of homicidal nurses and healthcare providers in relation to nurses and some physicians who committed serial homicides. A search of the LexisNexis database in the United States for criminal prosecutions of nurses and healthcare providers between 1970 and 2006 revealed 90 cases and indicated the following patterns: Nurses who were murderers were primarily white female nurses aged 40 or older. There were 3 to 4 times as many cases of serial murder by nurses than there were reports of suspicious deaths of patients where nurses were employed. The number of deaths of patients where nurses were reported as the caregiver ranged from a low of 0 to a higher of 50. The average number of suspicious patient deaths attributed to nurses where the killer was never arrested was 16.5, ranging from 2 to 50. The main method of killing was asphyxiation. The time period for victims ranged from a low of 6 months to a high of 10 years. Most murders of patients with records of suspicious deaths where nurses were the caregivers were never reported to any police agency or the FBI. The number of extraneous letters after the murderer's name by law enforcement was a high of 13 and ranged up to 27. The killers were never arrested, most of them received probation or served jail terms of a year. Some of the murderers were reported to have weapons and used lethal weapons including firearms, hands, and feet after committing their crimes. The number of homicides in which the healthcare provider was a nurse is 317 and the number of suspicious cases where the nurse was employed as a caregiver is 2113. It is felt that these numbers are alarming and demand that systemic changes in tracking adverse patient incidents associated with the presence of a specific healthcare provider be implemented. Hiring practices must shift away from preventing wrongful discharge or denial of employment lawsuits to protecting patients from employees who kill.
